Please give a breakdown of the items and their cost for which you are applying for. You will be expected to evidence the costs itemised above by including the relevant documents e.g. quotation, invoice, online quotation, details of projected spend. If your application is successful you will be expected to complete a brief monitoring report or film, and be able to provide evidence of the expenditure of the grant for the purposes applied for (e.g. receipts, bank statements, paid invoices). Failure to submit reports by the agreed monitoring dates may result in revocation of the grant monies.

The application contact MUST be able to attend the voting event on Saturday 23rd April 2022 from 11am – 1pm • Any grant made will be used ONLY for the purposes for which it is granted • Any unspent monies MUST be returned • I understand that failure to comply with the above requirements may result in the recovery of monies paid • Applicants MUST be willing to participate in any promotion in relation to the event • If you are under the age of 18, you have completed this application form with an adult. • Applications MUST be completed and submitted by 5pm Friday 1st April 2022.
